ENROLLMENT FORM

For the participation at the courses organized by TACTICAL ADVANTAGE it is requested to fill in the following form.

As the number of admitted participants for the courses is limited for each day, the registration forms will be taken in consideration regarding their date of arrival. First come - first serve. The organizer has the right to refuse to enroll candidates which are not conform with the course curriculum.

For the final admittance to the courses, the candidate has to pay the course fee on our bank account (info will be sent with the confirm mail). If the payment does not arrive within 10 days after the confirmation mail, the priority will go to another registration request.

The candidate also declares to understand and accept the following conditions:

  1. Because of logistic, organization and administration reasons, if the candidate can not participate to the course after his enrollment, (a possible substitute can be refused from the organizers) the entire fee will only be paid back if the request is done by writing no later than 15 days prior the class. 50% administrative fee will be assessed against a class tuition or attendance cancellations under 15 days prior to class.
  2. TACTICAL ADVANTAGE has the right to cancel the course in case of events which are out of their control or if the enrollment are not sufficient for an appropriate accomplishment of it. In this cases the enrollment fee will be fully paid back.
  3. The safety rules, the advices and the instructions of the instructors must be followed during the entire time of the course.
  4. The organizers and the instructors have the right to exclude a participant from the course if they found that the candidate does not follow their advice and/or when he/she puts him/herself and other persons with his/her conduct in danger.
  5. TACTICAL ADVANTAGE declines every responsibility for any accidents in relation to the practice of the trained shooting techniques (responsibility declaration form to be signed in on the day of the course).
  6. I herewith authorize after the Italian right about the privacy (D.L.vo 196/03) TACTICAL ADVANTAGE to handle my personal data only in relation with the courses.
At the right of the Italian law art. 1341 and 1342 c.c nn. 1), 2), 3), 4), 5), 6), I declare to accept the above conditions
